Complications - Open splenectomy

  1. Intraoperative complications

    • Injury to the tail of the pancreas: Atraumatic suture, drainage
    • Injury to the adrenal gland: Atraumatic suture
    • Injury to the colon: Oversew
    • Injury to the gastric wall: Oversew
  2. Postoperative complications

    • Postoperative bleeding: Early revision and hemostasis
    • Subphrenic abscess: Preferably CT-guided drainage
    • Pleural effusion: If it impairs breathing, drainage/aspiration
    • Pneumonia: Antibiotics
    • Thrombocytosis: Antiplatelet agents, if necessary
    • Pancreatic fistula: Drainage
    • Infections: Symptomatic, antibiotics
    • Portal vein thrombosis: Full heparinization, oral anticoagulants. Monitoring of at-risk patients in the first postoperative year by diagnostic Doppler ultrasonography and D-dimer testing
